Pre-Printed Chain of Custody Forms Arriving
Our laboratories, Medtox and LabCorp (LabCorp purchased Medtox for $241 million last summer), have been mailing out new chain of custody forms (CoC or CCFs) to your business or home. This unfortunately has been a rather long process. Once the new CCF’s arrive, please take a moment to verify the information and call us at 800-820-9314 or email if there are any changes or corrections needed. (cs@aadrugtesting.com)
Many have mistakenly perceived the arrival of the chain of custody forms as a notice to go and test. This is not the case; when the computer does Random selections we mail out the Random Notice along with the AADT Collection Site Passport – Form 111.
We ask that once the chain of custody forms have arrived that you store them until you receive a “random” notification from AADT to test. The new CCF’s can also be used for pre-employment tests for new hires, post- accident tests and reasonable suspicion tests.
The AADT Collection Site Passport – Form 111 is always required when going to test along with a chain of custody form so the proper procedures will be followed by the collection site. Failure to have the Collection Site Passport – Form 111 with you could cause the collection to be improperly collected or the collection site could bill you directly. Note: we also recently changed the AADT Collection Site Passport – Form 111 to be easier to read and follow by collection site intake personnel. Hopefully this will help to also reduce errors.

Many New Changes for 2013 – A Paperless Start
With the New Year we have implemented several new processes since going paperless.
- Renewals – If we have a fax number or email address on file, renewal forms are being faxed or emailed a month before expiration. If there are no major changes you can renew online as long as your enrollment hasn’t expired or you can call the office and pay with a credit card over the phone.
- Collection Site List – We will no longer be printing the collection site list, unless requested, to save paper costs. We now have available on our website the AADT Contracted Collection Site feature that allows you to search for a site by using a zip code.
- Profile Report – This will only be printed upon request, with online access to your account, you can print your profile report on your own at any time.
Once again, all the information in your Compliance Manual is available on our website.
